Sponsor, Funding and Governance:

Sponsor:

Accident and Emergency Department, Hospital Selayang and the
Neurology Department, Hospital Kuala Lumpur.

Funding:

The CRC of the MOH will provide technical support in the form of clinical epidemiology expertise, and biostatistics and ICT services.

Advisory Committee:

An Advisory Committee will be established to govern the NTrD.
The MOH, various professional bodies, and providers of emergency and head injury related services) from all sectors will be represented on this committee to ensure that the NTrD stays focused on its objectives, and to assure its continuing relevance and justification.

Objectives:

The objectives of NTrD are to:

  1. To determine the frequency, mechanism of injury and
    distribution of major trauma in Malaysia. These are useful
    measures of health burden arising from the major trauma
    and its management in the country.

  2. To determine the outcome and probability of survival of the trauma patients.

  3. To evaluate the major trauma management in the participating hospitals and to come up with guidelines for improved trauma care.

  4. To determine the extent to which improvements
    introduced, have been achieved.

  5. To stimulate and facilitate research on major trauma and its management.

Scope and Content:

The NTrD is intended to be a truly national population-based registry. It therefore seeks the participation of all emergency and head injury care providers from all sectors (public, NGO and Private) throughout the country.
Apart from basic demographic and baseline clinical data, NTrD also collects data on outcome of trauma patients. The injuries of the trauma patient will be scored according to the ISS (Injury Severity Score) Method. The head injury patients will have their outcomes data monitored at discharge, 3 months , 6 months and 12 months.

Data collection and transfer:

The NTrD will be collecting data using mix mode method ie it will be paper and web application based to capture data.

Record linkage:

There is a plan to link the NTrD database to the:
• Jabatan Pendaftaran Negara’s database to ascertain mortality outcome.
• Transplant registry database to ascertain transplant outcome.

There is also a plan to link the NTrD to the National Operative Procedure register and the MOH’s Health Management Information System (HMIS) to determine morbidity events when these two databases are properly established.

Reports & Publications:

The NTrD will be publishing its annual statistical report in April 2007.
NTrD will be looking into variation in certain treatment and outcome measures among emergency and head injury providers. An individual centre’s result will be compared against the national average or some other reference values to identify potential opportunity for quality improvement. Such centre specific reports however, will be strictly for the consumption of the individual centre and will only be produced if specifically requested by a centre.

Resources:

Registry operations:

The day-to-day operations of the NTrD will be undertaken by NTrD staff, which will comprise of the paramedic team in Hospital Selayang and in Hospital Kuala Lumpur.

Epidemiology:

The Clinical Research Centre of the MOH will provide clinical epidemiology expertise for the registry.

Biostatistics:

This is outsourced to Biostat Consulting Sdn Bhd.

ICT support:

These are outsourced to various IT companies including NTT (data centre service), MIMOS and KSM (hardware), Datamed Clinical Computing Sdn Bhd (databases and applications), SecureTangent (information security), and variety of specialty software providers (IBM/Ascential, SAS, STATA, WHO/MedDRA, etc)
